Providence Counseling Services will verify your insurance benefits so that you are aware of any part of your responsibility surrounding your sessions, which may include your copay or deductible.  

Questions you may ask your insurance company in advance:

• Do I have mental health benefits?
• Do I have a deductible and has it been met?
• Do I have a copay and if so, what is the copay amount?
• Do I have a certain number of sessions allotted per year?
• Do I need a referral from my primary care physician?

• Quest Behavioral Health
• United Healthcare
• United Healthcare Medicare Advantage

*We also work with numerous EAP companies so please feel free to give us a  

  call to determine if your particular company is covered.

Individual Session (50-60 Minute Session): $125.00

Couples Counseling Session (50-60 Minute Session): $125.00

Family Session (50-60 Minute Session): $125.00

We ask that you notify us with at least 24 hours in advance to cancel your appointment as late cancellations and no-shows will be required to pay a cancellation fee.  

Cash, checks and all major credit cards are accepted for payment.
